4/1/2024 EnchantaVaughn Approved Upon arrival to the home setting, Client stated that she completed her chores and her homework after school without giving her mom a hard time. Client appeared to be in happy mood as evidenced by Client smiling as she greeted QMHP-C and verbalizations throughout as well as mother. Mother reports client continues to struggle with managing her anger on a daily basis at which behaviors can last up to 10 minutes in duration when she does not have her way. Behavior: Client was sociable and friendly in regard to QMHP’s positive empathy. Client did not need any redirection from QMHP-C while QMHP-C encouraging client to continue to implement learned techniques to assist her with reaching her ISP goals while in services. Intervention: QMHP effectively utilized tone of voice, eye contact, body language while Client expressed her point of view. QMHP modeled the language and actions Client is expected to give and receive from others. QMHP prompted Client to mentally reverse roles with Mom to show her how she made her feel. QMHP continues to build a relationship with Client by encouraging Client to express her thoughts in an open and confident way. Intervention: QMHP provided both parties with positive feedback. QMHP asked questions along with the allowing both parties to ask questions. QMHP praised Client for her positive behavior in the home. QMHP developed a rewards system in which parent is to reinforce the Client’s expression of her anger in a controlled manner. QMHP-C encouraged the Client to continue to display positive behaviors in the home. Response: Client cooperated with QMHP-C during role-play and communicated that she understands how her past behaviors have negatively affected her. Client’s guardian communicated with QMHP-C that she likes the idea of engaging in role-play with Client because she feels as if Client would better understand negative versus positive by actually seeing it first hand from someone that means a lot to her, as well as, help her engage with her daughter more. Response: The mother indicated that she is willing to assist in any way she can. Client was not receptive to the idea of trying to engage in positive activities with her mom. Client stated that she does not intentionally “act up” and wants to behave better and treat her mom with respect. Client was able to participate in the session while implementing positive behaviors.
